Waiver and Release of Liability for Participation in Brazilian Jiu-Jitsu Classes

I, {name} , hereby acknowledge and agree to participate in Brazilian Jiu-Jitsu classes at RISE Brazilian Jiu-Jitsu Academy Belgium (the "Academy"). In consideration of being allowed to participate in these classes, I understand and acknowledge the following:

1. Assumption of Risk: I am aware that Brazilian Jiu-Jitsu is a full contact sport involving physical exertion, grappling, throws, holds, and ground fighting techniques, which may result in personal injury, accidents, or even death. I understand that certain risks are inherent in participating in these activities and that human error cannot always be excluded, even though the instructors make every effort to create a safe training environment.

2. Physical Condition: I hereby represent that I am in good physical condition and have no medical conditions that would prevent me from safely participating in Brazilian Jiu-Jitsu classes. I acknowledge the importance of consulting a physician prior to engaging in any physical activity if I have any concerns about my physical health.

3. Waiver and Release: In consideration of being allowed to participate in Brazilian Jiu-Jitsu classes at the Academy, I hereby release, waive, discharge, and covenant not to sue the Academy, its owners, instructors, employees, and any other affiliated individuals or entities from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, illness, or death, that may be sustained by me or any property belonging to me, whether caused by the negligence of the Academy, its instructors, employees, or otherwise.

4. Indemnification: I agree to indemnify, defend, and hold harmless the Academy, its owners, instructors, employees, and any other affiliated individuals or entities from and against any and all liabilities, claims, demands, actions, or causes of action, costs, and expenses, including reasonable attorney's fees, arising out of or in connection with my participation in Brazilian Jiu-Jitsu classes at the Academy.

5. Use of Likeness: I grant the Academy the irrevocable right and permission to use photographs, video recordings, and any other media in which my likeness appears, for promotional or educational purposes, without compensation or approval rights.

I HAVE CAREFULLY READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS CONTENTS, AND VOLUNTARILY AGREE TO ITS TERMS. I UNDERSTAND THAT BY SIGNING THIS DOCUMENT, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE
